Pharmacy Residency Brought to ACMC

ACMC Health, in collaboration with Cash Wise Clinic Pharmacy in Willmar, is excited to host its first Pharmacy resident. This residency, which is focused on Ambulatory Care, is being offered by the University of Minnesota. An Ambulatory Care pharmacist has the advanced knowledge and expertise to focus on the special needs of patients who have conditions requiring multiple medications who often administer these drugs themselves or with the assistance of a caregiver at home. These pharmacists also educate and engage patients in health promotion and wellness; this includes helping manage patients in between provider visits through treatment assessment, monitoring compliance, refilling prescriptions and patient education.

Gunjan Shah is a licensed Pharmacist who has been working with Cash Wise Clinic Pharmacy and ACMC providers to deliver a high level of care to patients. She is working under the supervision of Julie Hansen, an experienced clinical pharmacist. On top of being a preceptor for this program, Julie has also taken a role of a member of the Residency Advisory Committee at the University of Minnesota. Her goal is to create and maintain a residency program that nurtures new graduates of pharmacy programs and allows them to grow into a competent clinical pharmacist.

The University of Minnesota’s residency program is considered one of the most prestigious in the country, with the pharmacy program ranking second in the nation. From the pharmacists who applied nationwide, Gunjan Shah from Rochester, New York was selected to become the first resident of ACMC-Cash Wise Pharmacy program. Gunjan is originally from India and graduated pharmacy school through the University at Buffalo. “This is a brand new program,” Gunjan explained. “This is an excellent opportunity for me to learn from Julie and successfully establish an ongoing ambulatory care focused program.”
Gunjan admits she was hesitant to move from New York, but says that the people at ACMC and Cash Wise Clinic pharmacy have made this transition easier. “I can’t believe how we all work so well together,” Gunjan said. “Providers know my name and are quick to respond. The teamwork in the system is astounding.

Gunjan meets individually with ACMC patients to discuss their medications in a comprehensive manner. The sessions allow her to assess indication, safety, efficacy and convenience (cost) of each medication on a patient to patient basis. This program is capable of providing and managing smoking cessation, chronic disease management, addressing drug therapeutic problems including interactions and adverse effects, and medication adherence issues. Currently, the program is in the process of developing and signing a Collaborative Practice Agreement between ACMC providers and Cash Wise Clinic Pharmacy to combat Tobacco Cessation.  

Together, Gunjan and Julie are learning how providing Medication Therapy Management can be a valuable resource for ACMC, its’ providers, and their patients.
